- to be set on publication
- Draft prepared
- August 27, 2026
1. Who This Notice Covers
Pioneer AI Health ("we," "us," or "our") is a Georgia-based, veteran-led company headquartered at 1050 Crown Pointe Parkway, Suite 539, Atlanta, GA 30338. This Notice applies to protected health information (PHI) handled by:
- Pioneer AI Vitals — pioneeraivitals.com — in connection with its remote patient monitoring program, personal health record, and related clinical data services.
This Notice governs protected health information handled within a clinical or care-coordination relationship. General website, newsletter, and marketing data are instead governed by our separate Privacy Policy. Where the two documents conflict on the handling of PHI, this Notice controls.
2. Our Legal Duty
We are required by law to maintain the privacy of your protected health information, to provide you with this Notice describing our legal duties and privacy practices, to notify you following a breach of unsecured PHI, and to abide by the terms of the Notice currently in effect.
3. How We May Use and Disclose Your Health Information
Depending on the program you're enrolled in, we use and disclose PHI for the following purposes without requiring your separate written authorization:
3.1 Treatment
We may use and disclose your health information to provide, coordinate, or manage your care and related services — for example, sharing remote-monitoring readings with the healthcare provider or care team treating you.
3.2 Payment
We may use and disclose your health information to obtain payment for services, including sharing information with your health plan or the healthcare organization coordinating your billing for remote patient monitoring services.
3.3 Health Care Operations
We may use and disclose your health information for our own or a treating organization's health care operations — for example, quality assessment, care coordination, staff training, and program evaluation.
3.4 Other Permitted Uses and Disclosures
We may also use or disclose your health information, without your written authorization, when:
- Required by law, such as in response to a court order or subpoena.
- Necessary to prevent or lessen a serious and imminent threat to your health or safety or that of others.
- Requested by a public health authority, to report disease, injury, or as required for public health surveillance.
- Requested by a health oversight agency for audits, investigations, or licensure activity.
- Required for organ or tissue donation purposes.
- Necessary to comply with workers' compensation laws.
- Shared with a business associate who performs services on our behalf under a signed agreement requiring them to protect your information.
- Shared with the U.S. Department of Veterans Affairs or other government health programs when you have separately authorized or requested that coordination.
5. If You Were Enrolled Through a Healthcare Organization
If you're using Pioneer AI Vitals as part of a program offered by a hospital, clinic, or other healthcare organization, that organization is your treating provider and has provided you with its own Notice of Privacy Practices. This Notice describes how Pioneer AI Vitals itself handles your information as part of delivering the remote-monitoring service; it does not replace your treating organization's notice.
6. Your Rights
You have the following rights regarding your protected health information. To exercise any of these rights, contact us using the information in Section 10.
- Right to Access — request a copy of your health record, in paper or electronic form.
- Right to Amend — ask us to correct information you believe is incomplete or inaccurate.
- Right to an Accounting of Disclosures — request a list of certain disclosures we've made of your health information.
- Right to Request Restrictions — ask us to limit how we use or disclose your information; we are not required to agree, except where you paid out of pocket in full and ask us not to disclose to your health plan.
- Right to Confidential Communications — ask that we contact you by a particular method or at a particular location.
- Right to a Paper Copy — request a paper copy of this Notice at any time, even if you agreed to receive it electronically.
- Right to Notice of a Breach — be notified if unsecured PHI about you is breached, as required by law.
- Right to Choose a Personal Representative — have someone you've authorized act on your behalf regarding these rights.
7. Substance Use Disorder Records
Pioneer AI Vitals does not currently operate a federally assisted substance use disorder treatment program subject to 42 CFR Part 2. If that changes, we will issue a separate Part 2 Notice of Privacy Practices describing the additional protections that apply to those records, consistent with the Part 2–HIPAA alignment rules that took effect February 16, 2026.
8. Changes to This Notice
We reserve the right to change this Notice and to make the revised Notice effective for health information we already have as well as information we receive in the future. The current Notice will always be available on our website and, on request, in paper form.
9. Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
